San Francisco Unified School District World Wide Web Parental Permission

The San Francisco Unified School District ("District") publishes and maintains a World Wide Web Site ("Web Site") on the Internet. The purpose of the Web Site is to provide easy access to District information and services. A Web Site is a location on the Internet. The Internet is a network that connects personal computers from around the world. The District Web Site can be accessed at < www.sfusd.edu >. Parents, students and District employees may access the Web Site. In addition, any other person with a computer connected to the Internet may access the Web Site.

I ___________________________ understand that the attached photograph of (Parent or Guardian please print) my child or schoolwork completed by my child ____________________________ (Name of Child please print) will be used on the District Web Site for a period not to exceed one year. I understand that this material can be viewed by anyone with access to the Internet. I also understand that the District will do its best to ensure that my child’s photograph (which may appear larger when viewed on a computer screen) or schoolwork will NOT be accompanied by my child’s name or with any other text that could reveal the identity of my child.
